How does the program work?
A panel of judges will review nominee forms. Individuals showing outstanding effort in promoting donation and demonstrating great compassion in supporting families through the donation process will be selected to receive the "Yes" Utah Award.
Who are the judges?
The judges are selected from members of the Utah's Donate Life Coalition.
How can I nominate someone for the award?
Complete and submit the award nomination form on line or call to receive a form from Utah Lions Eye Bank at (801) 581-2039 or Intermountain Donor Services at (801) 521-1755.
Who is eligible to be nominated?
Anyone who facilitates donation, such as nurses, doctors, medical staff, chaplains, funeral directors, social workers, or anyone who has shown great compassion in supporting families through the donation process.
Who can nominate?
Anyone who witnesses the donation event and then completes and submits the award nomination form.
What will be awarded?
Award recipients will receive a certificate of accomplishment and a "Yes" Utah clock & pen gift set. To nominate a Utah professional for a "Yes" Utah Award, click here.
